90 245 shakey idle

My 245 has had a shakey idle since went it 6 years ago, over that time the O2, injectors, plugs, wires, distibutor cap/rotor, maf, fpr, ecu and ezk have all been swaped, just as maintenance, not ever trying to fix the issue. Well I've finally gotten tired of it, and want to smooth it out, but I can't think of what else to check. It pulls hard, no cold start issues, the fuel economy is low for what it should get, ~22mpg freeway when my turbo gets 24 freeway.

 
Have you made sure your cam timing is correct? Have you done a valve adjustment? Those injectors are not correct for your motor, I ran some "modern" 4 hole injectors and it didn't run better than cleaned and flow tested yellow pintle cap LH2.4 injectors. I have also found a noticeable improvement in idle quality by using BP6ET spark plugs, here's my '90 with an H cam idling for example.
 
Next step for me would be intake manifold gasket. You might want to be sure compression readings are similar among the four cylinders before taking the step to check valve clearance.

Looks like intermittent miss or weak combustion. Which cylinder is missing? Then, what does the spark look like and what's the compression? Is the fuel flow equal to all cylinders? Compare with other cylinders. Record some data for us.

This, if you have never replaced the intake gasket. They almost always crack at the bottom of the port which makes a water spray useless in diagnosing a leaking gasket.
 
Intake gasket has been changed, cam timing is correct, though it does have an A cam, and I need to check the clearance, though it did it with the m cam before. Changing the injectors made no difference either, though I have gotten better economy with these ones.
 
You went from an M to A and not adjusted valves ?

No extra ticking, audibly checked, and everything I have heard is going from volvo to volvo cam it rarely needs shimming. I know I need to, but I don't have a shim kit so I couldn't do anything anyway. Next time Anthony comes over I will make him bring the kit
