Does machining ~1mm off the B230 head result in poor idle quality (due to late camshaft events)?

Indeed. Cam STS adjustable pulley calculates out to ~ .035" per cam degree.... the matching 2 degrees crank.
 
The real question is did you get the results you were after
Idle vacuum improved, as well as idle quality. This came at the cost of some lost torque the A cam provided.

My previous cam & timing setup may have been the cause of my continued random CEL codes thrown for "knock sensor deficient or missing" in my REX/Regina system. This remains to be seen, as I've got the 745 down for an AC overhaul (a necessity in the hot humid SE region). Once I get some run time on the T, we shall see if that along with advancing the timing back to stock, resolves the REX knock faults.
